Aboriginal Chronic Care Unit – Nowra – Key programs and services

  • Shoalhaven Aunty Jean’s Chronic Disease Program
  • Ulladulla Aunty Jean’s Chronic Disease Program
  • Facilitation of outreach services to Jervis Bay Territory (Wreck Bay community and Jervis Bay Village) and the Orient Point (Jerrinja) community
  • Post-48 hour Follow-up service
